General Guidelines

  • Please take the time to review all criteria prior to scheduling a patient to avoid scheduling a patient who is not a candidate for sedation.

  • Please review ALL patient instructions with patient BEFORE scheduling an appointment. Patient must complete IntakeQ questionnaire and receive clearance from Selkirk Anesthesia before scheduling an appointment.

  • When completing the financial agreement form the office will insert the negotiated hourly rate as well as the estimated amount due day of services section.

  • If you don't see a day on our availability calendar please contact us to see if we can accommodate your desired day/time.

  • For any pediatric patients please contact us prior to scheduling an appointment.

Patient Selection Criteria

Selkirk Anesthesia  reserves the right to approve or deny any case for office-based anesthesia for any reason that the provider rendering anesthesia deems unsafe for a patient to undergo anesthesia in the office setting. The following criteria are in no way all-encompassing, and each case will be approved on a case-by-case basis. ASA 1, 2, and 3 patients will generally be acceptable candidates, ASA 4 patients will not typically be considered for office-based anesthesia. The following are potential reasons that a patient would not be considered a candidate for office-based anesthesia.

  • BMI greater than 35 (use a BMI calculator to determine)

  • Uncontrolled hypertension

  • Aortic stenosis, mitral stenosis, other heart valve issues

  • Poorly managed chronic heart failure

  • Recent myocardial infarction or stroke

  • Pulmonary hypertension

  • Recent blood clot or pulmonary embolism

  • Severe asthma

  • Uncontrolled diabetes

  • Kidney failure requiring dialysis

  • History (or family history) of Malignant Hyperthermia

  • Uncontrolled seizures

  • History of anaphylaxis

  • Past complications with anesthesia

  • Known or anticipated difficult airway

  • Noncompliance with NPO guidelines

Patient Instructions

***Patients must follow instructions or they are at risk of having their case canceled due to non-compliance***

  • Patient MUST fill out and return pre-anesthesia history and evaluation and be approved by the anesthesia provider before Selkirk Anesthesia LLC will schedule a case.

  • Plan to receive a phone call from your anesthesia provider sometime during the 1-2 weeks before your scheduled procedure. Failure to talk with anesthesia provider over the phone before the day of the procedure may jeopardize the scheduled procedure and necessitate a cancellation.

  • Payment must be collected prior to the day of the procedure if the patient will be making a payment directly to Selkirk Anesthesia LLC. If payment is not collected prior to procedure no anesthesia services will be rendered.

  • Nothing to eat or drink for at least 8 hours prior to receiving anesthesia

  • Take necessary medications with small sip of water and only take medications that have been discussed with anesthesia provider previously

  • If patient is on Ozempic (or other GLP-1 agonist meds), this medication MUST be held for at least one week prior to the case. Patient must also be on a clear liquid diet for the 24 hours before the case.

  • Must have a ride home from a responsible adult and a responsible adult who can stay with them overnight for the first night after their procedure.

  • Plan to not return to work for 24 hours after receiving anesthesia

  • No operating any kind of machinery for 24 hours after receiving anesthesia

  • No consuming alcohol for at least 24 hours after receiving anesthesia

How To Book A Case

  1. Patient must complete intakeQ form accessed through the button on the Selkirk Anesthesia website. This is a health history as well as a financial agreement. If more information is requested by Selkirk Anesthesia the patient or office must provide this requested information.​

  2. Office will deliver each patient a copy of the estimated amount due for anesthesia services using the financial agreement form. Please allow 30 minutes of setup and recovery time when giving the estimate. (Ex:if procedure will be 2 hours long please quote 2 hours and 30 minutes of anesthesia time)

  3. Once all forms and information have been submitted to Selkirk Anesthesia the patient will typically receive clearance (or denial) within 1-2 business days.

  4. Once clearance for anesthesia is given an appointment may be booked using the scheduling calendar on the Selkirk Anesthesia website. At the time of booking a $500 non-refundable deposit must be collected with the help of the office scheduling the appointment. 

  5. Please verify the day being booked is with a provider who is licensed in your state. This can be viewed under provider info within the scheduling calendar.

  6. When booking, please inform Selkirk Anesthesia of the desired start time for the procedure, what the procedure is, and the estimated length of the procedure. This information can be placed in the notes section of the booking. We typically recommend having the patient arrive 45 minutes prior to the desired start time. Please also allot at least 1 hour before the patient arrives for Selkirk Anesthesia to access office space and setup equipment, occasionally we may ask to come setup the night before.

  7. Patient will receive a phone call from Selkirk Anesthesia 1-3 weeks prior to review their health history, medications, the plan for anesthesia, financial arrangements, and pre-anesthesia instructions. Failure to talk with Selkirk Anesthesia will result in a cancellation and forfeiture of $500 deposit.

If patients are making payment directly to Selkirk Anesthesia they will pay via the pay link on the website to place their $500 deposit.If Selkirk Anesthesia will be invoicing the office for services, the office will collect the $500 deposit. If a case is cancelled Selkirk Anesthesia will invoice the office the amount of the $500 deposit and it will be due the day it is received.
